Arkansas Department of Education/Child Nutrition Unit (ADE/CNU) is reducing the required paperwork and clarifying the previous Commissioner’s MemoFIN-13-034 “Additional Six Cents Performance Based Reimbursement for School Lunches.” After clarification with Southwest Regional Office of the United States Department of Agriculture (USDA), ADE/CNU is amending the documentation required for submission for the additional 6 cent certification.  NOT REQUIRED: With the submission of the Nutrient Analysis or the USDA Simplified Nutrient Assessment Tool, districts will not be required to submit: ·        Recipes ·        CN Labels or Manufactures Product Analysis Sheet or Formulation Statement ·        Nutrition Fact Labels – including the products ingredient list REQUIRED: All other documentation requested for Option 1 or 2 from Commissioner's Memo FIN-13-034 is still required.  Option 1: 1.      Six Cents Certification Application Cover Page 2.      5 Day-Week of menus for breakfast and lunch menu types offered, by age grade group, with portion sizes listed for each menu item 3.      USDA approved menu planning worksheet (submitted on a CD or disc) (please do not print this document) due to formatting problems that have not been addressed by USDA 4.      Nutrient Analysis of 5 Day-Week of menus 5.      Attestation Statement signed by district officials  Option 2: 1.      Six Cents Certification Application Cover Page 2.      5 Day-Week of menus for breakfast and lunch menu types offered , by age grade group, with portion sizes listed for each menu item 3.      USDA approved menu planning worksheet (submit on a CD or Disc) 4.      USDA Simplified Nutrient Assessment Tool (submit on a CD or Disc) 5.      Attestation Statement signed by district officials ADE/CNU will review the recipes, CN Labels or Manufacture's Product Analysis Sheet or Formulation Statement and Nutrition Fact Labels if submitted by a District with the six cent certification documentation.   VALIDATION REQUIREMENTS: Districts are reminded that production records along with the recipes, CN Labels or Manufacturer's Product Analysis Sheet or Formulation Statement and Nutrition Fact Labels will be reviewed during the on-site validation review.
